Address Change Request pdf   Complete this form to update your address for official transcript processing. Prior students ONLY. Current students use Orion for updating addresses.
Change of Admissions Application Request pdf   This form can be used to change a plan on an admissions application for a particular semester. A student can only change an application term forward or back one long semester (fall or spring). The request must be filed by Census Day of the current application term.

Credit/No Credit Request pdf website For undergraduate students only. Undergraduate students must complete form and turn in to the Office of the Registrar in the Student Services Building before census day of the given term.
Diploma Re-order   website Students who graduated from UT Dallas who wish to order additional copies of their diploma may re-order through the UT Dallas Marketplace.
Doctorate Change of Program pdf   For Doctoral students only. Request form for changing program within the same school.
Embassy Letter Request pdf   For students who applied to graduate in the current term only. The purpose of this form is to allow graduating students to invite their family to the commencement ceremonies at the end of the semester.

Students without a social security number and who have elected to withhold their name and dates of attendance may not use the online service. Please fill out the provided form.

Students needing enrollment certifications for auto insurance, scholarships and health insurance may not use the online service. Please fill out the provided form.

Examine Student Record Request pdf   Students complete this form and return to the Office of the Registrar. Information may not be available on demand; therefore, an appointment may be scheduled for the examination of your records.
Exemption to Excessive hours policy pdf  

For undergraduates only. For undergraduates who are being assessed additional tuition for excessive attempted hours. Complete this petition to request a one-semester exemption from Excess Hours tuition. This petition is due in the Office of the Registrar by the full-term census day of the semester for which the exemption is sought.

FERPA Packet pdf   Request packet includes: Request for Confidentiality, Parents Certification of Dependency, and Third Party Release Request
Graduation Verification Letter pdf   This form is for requesting a letter that will state that the student has earned a degree from the University of Texas at Dallas.
InAbsentia Forms - Undergraduate and Graduate pdf   Applying to graduate in absentia allows a candidate to apply to graduate without formally enrolling in courses for that semester. The .pdf contains both the undergraduate and graduate in absentia forms. Please complete the form appropriate to your career.
Intent to Participate Form pdf website This form is required for all summer graduation applicants.

Meningococcal Meningitis Vaccination Requirement Form pdf website

Per new State legislation beginning January 1, 2012, all entering Texas college students must receive a vaccination or booster (if the vaccination is five years old) against bacterial meningitis before enrollment.

Complete the form and supply the necessary documents to the Office of Enrollment Services in the Student Services Building no later than 10 working days before the semester begins. Entering students will be unable to register until the paperwork is received and reviewed.

The Spring 2012 deadline is January 6, 2012.

Students may fax or mail the form and supporting documentation.

Petition for Graduate Deferment of Admission

  A statement of reason and any supporting materials the applicant believes to be relevant and wants the Academic Program to consider should be submitted with this form.
Petition for Undergraduate Deferment of Admission pdf   The deferment is only valid for the semester of the student's original application. Admission can only be deferred for one semester unless the student is a member of the Transfer Comet Connection program. Deferment request must be filed by Census Day of the original application term.
Petition for Undergraduate Student to Take Graduate Course pdf website For undergraduate students only. Undergraduate students enrolling in graduate courses must complete this form.
Petition to Use Military Credit pdf   Complete this form to petition awarding academic credit for military service. Please include the appropriate documentation with the form and submit to the Office of the Registrar by Census Day.
Petition to Use National Test Scores as Transfer Credit pdf website For undergraduate students only. Undergraduates students complete this form to receive transfer credit from appropriate test credit. Please review the undergraduate dean's website for details regarding test credit that applies.
